FinalForms

Newark City Schools uses FinalForms for all student forms — registering new students, back-to-school forms, and athletic forms. All parents should have an account. If you do not have an account for your child, please contact your child's school to ensure the school has your correct contact information.
Click here to visit FinalForms and fill out your child's online forms. Online forms must be filled out once per school year to update any information that may be changed, preferably by the first two weeks of school. These forms include Emergency Medical information, which must be signed prior to your student attending any field trips.
If any information changes during the school year — especially phone numbers, email addresses and physical addresses — you can now make these changes via FinalForms at any time. Please log into your account to make the changes, then contact your child's school to let them know you made that change. Contact information is important to keep updated so that the school can contact you in an emergency and also contact you via ParentSquare.
